Class members who submit a valid claim form will receive a cash payment of $20.00. However, the amount could vary depending on the number of valid claims submitted.
To file a claim, you can either submit a claim form online or download and mail a completed form to the settlement administrator. Your claim must be submitted online or postmarked by January 27, 2025.
You will need to provide proof when submitting your claim form. If you received a personalized notice with a Notice ID and Confirmation Code, be sure to enter these codes when filing your claim.
