Total Settlement Amount

How Do I Qualify for a Payout?
You may qualify if you are or were a patient of DaVita and visited any of its websites, patient portals, or mobile applications at least once between November 20, 2017, and September 21, 2023.
How Much Can I Get Paid?
The cash payment amount will be determined by dividing the Net Settlement Fund (after deducting Privacy Shield costs) by the number of valid claims. Additionally, you can choose to enroll in one year of Privacy Shield services.
How Do I File a Claim?
To file a claim, submit a claim form online or by mail, postmarked by December 4, 2024. Claim forms are available on the official settlement website or by contacting the DaVita class action administrators. You will need your Member ID, which can be found on any notice you received by mail or email.
Lost or Misplaced Notice?
