Eligibility Criteria

Potential Compensation
Class members may be eligible for the following:
- Reimbursement for documented out-of-pocket losses related to the data breach, up to $1,000.
- Cash payment on a pro rata basis, estimated at $50 but capped at $300. The exact amount will depend on how many valid claims are submitted and remaining settlement funds.
- Two years of credit monitoring and identity protection services provided by a credit bureau. Individuals who previously accepted free monitoring from Crossroads may still claim this benefit.

Documentation Requirements
- For reimbursement of out-of-pocket expenses, documentation is required. Acceptable documents include credit card or bank statements, invoices, and receipts that link the expenses to the breach. Personal account details may be redacted as necessary.
- Online claims require a unique ID and PIN found on the notice postcard. Those who did not receive or lost the postcard should contact the settlement administrator at 888-820-1005.
