Note: If claims exceed $200,000, individual payments may be reduced proportionally.
2. Alternative Pro Rata Cash Payment:
If you do not claim specific losses, you may request a pro rata cash payment. The estimated amount is around $50, but the final figure depends on how many people file valid claims and the remaining settlement funds after deductions.
3. Medical Account Monitoring:
All claimants—regardless of cash choice—can opt in for one year of free medical account monitoring, which includes:
- Dark web monitoring
- $1 million in medical identity theft insurance
- Real-time alerts and high-risk transaction monitoring
- Security freeze and victim assistance services

- Online, using the unique ID and PIN provided in your mailed notice
- By mail, using a printed PDF claim form
