Overview:
Greater Cincinnati Behavioral Health Services (GCBHS) has agreed to an $850,000 settlement to resolve a class action lawsuit stemming from a data breach that occurred in December 2023. The breach allegedly compromised sensitive personal and medical information. Affected individuals may be eligible to receive up to $5,000 for documented losses, a cash payment estimated between $60.90 and $131.48, and/or one year of identity theft and credit monitoring protection.
Who Is Eligible?
To qualify as a class member, individuals must:
- Be a resident of the United States
- Have received a notice from GCBHS or the settlement administrator regarding the December 2023 data breach
The breach may have exposed information including:
- Full names
- Dates of birth
- Social Security numbers
- Driver’s license or state ID numbers
- Health insurance policy or plan numbers
- Medical and health-related data such as:
- Medicare/Medicaid numbers
- Patient and medical record numbers
- Provider names
- Treatment locations and dates
- Diagnoses and treatments
- Prescriptions, medical histories, and test results
An estimated 61,850 individuals are included in the class.
What Compensation Is Available?
Eligible class members may submit claims for the following benefits:
1. Out-of-Pocket Loss Reimbursement (up to $5,000):
Class members may request reimbursement for documented expenses related to the breach, including:
- Costs due to identity theft or fraud involving a Social Security number
- Fees for accessing, freezing, or unfreezing credit reports after December 2023
- Postage, notary services, copying, mileage, and other related costs
- Lost wages or unpaid time off spent responding to the breach
- Other expenses directly linked to the data incident
2. Cash Payment (estimated $60.90 to $131.48):
Available to all class members regardless of whether they experienced financial loss. The final payment will depend on the number of approved claims and the amount left in the fund after expenses.
3. Identity Theft and Credit Monitoring:
All class members are eligible for one year of CyEx Medical Shield three-bureau credit monitoring and identity protection.

- By mail using a completed claim form from page 45 of the settlement agreement
