Imagine360, LLC, a provider of services to employee health plans, has agreed to a $475,000 settlement following a data breach that exposed sensitive personal information. If you received a notification stating that your data was affected in the January 30, 2023 incident, you may be eligible for compensation.
Who Is Eligible?
You’re considered a class member if:
- You live in the United States, and
- You received a notice from Imagine360 stating your information may have been compromised during the January 30, 2023 data breach.
Settlement Benefits and Payment Options
Eligible individuals can choose one of the following cash payments and also opt for free credit monitoring:
- Documented Losses (Up to $5,000): Reimbursement for verifiable out-of-pocket expenses directly tied to the data breach. Examples include bank fees, phone/data charges, gas for local travel, or costs for identity theft protection services. Receipts and official documentation are required—personal statements or affidavits are not sufficient.
- Flat Cash Payment (Approximately $75): For those not submitting documentation. Final amount depends on the number of claims filed.
- Credit Monitoring: Up to three years of complimentary credit monitoring for all class members, regardless of the cash payment selected.
How to File a Claim
Claims must be submitted by July 31, 2025. There are two options:
- Online: File through the official settlement website using your Unique ID provided in the notice.

Payment Methods
Approved claims will be paid either via:
- Electronic transfer, or
- Check by mail, sent to the address on your claim form.
Settlement Fund Allocation
The $475,000 fund will cover:
- Administrative costs
- Attorneys’ fees (up to $166,250)
- Litigation costs (to be submitted for court approval)
- Class representative service award (up to $2,000)
- Credit monitoring costs
- Payouts to approved class members (balance of the fund)

When Will Payments Be Made?
If the court approves the settlement on August 15, 2025, payments to approved claimants are expected to be issued within 75 days of that approval.
Why Was There a Settlement?
The settlement stems from a class action lawsuit following a data breach of Imagine360’s Citrix file-sharing system, which exposed names, Social Security numbers, health insurance data, and medical information. The plaintiffs claimed Imagine360 failed to properly safeguard this information.
While Imagine360 denies any wrongdoing, the company agreed to settle in order to avoid further litigation costs and to resolve the claims.
