Rancho Family Medical Group $315K Data Breach Settlement

Current and former patients whom Rancho Family Medical Group notified about a November 2023 data breach involving service provider KMJ Health may be eligible to receive up to $10,000 for out-of-pocket losses and three years of credit monitoring under a class action settlement.

Rancho Family Medical Group agreed to pay $315,000 to settle a lawsuit alleging it failed to adequately safeguard patient information during the KMJ Health Service data breach in November 2023.

Who Can File a Claim?

Eligible class members are U.S. residents who received a notification from Rancho Family Medical Group stating that the November 2023 KMJ Health data breach may have compromised their personal and protected health information.

How Much Can Class Members Receive?

All class members automatically receive:

Pro rata cash payment (up to $1,000 per person)

Three years of credit monitoring and identity theft protection

  • Provided via a redemption code included in the notice.
  • No claim form is required if enrolling before the stated deadline.

Class members may also submit a claim for additional compensation:

Out-of-pocket losses (up to $10,000)
Eligible documented, unreimbursed expenses related to the breach may include:

  • Fraud- or identity-theft-related losses
  • Fees for attorneys, accountants or other professionals
  • Costs to freeze or unfreeze credit
  • Post-breach credit monitoring expenses
  • Miscellaneous expenses such as notary fees, postage, copying or mileage
    Supporting documentation is required.

Attested time (up to $68)

  • Calculated at $17 per hour for up to four hours.
  • Covers tasks such as updating passwords, monitoring accounts, contacting institutions or researching the breach.
  • No documentation is required beyond attestation.

How to File a Claim

To request reimbursement for out-of-pocket losses or attested time, class members can submit an online claim form or download, print, complete and mail the PDF claim form.

Claim deadline: December 29, 2025

A claim form is not required to receive the automatic pro rata cash payment or the credit monitoring benefit.

Why Is There a Settlement?

The lawsuit alleged that KMJ Health Service, a vendor for Rancho Family Medical Group, experienced a breach that exposed patients’ personal and protected health information. The plaintiff claimed Rancho Family Medical Group failed to adequately protect this information. Rancho Family Medical Group denies the allegations but agreed to the settlement to avoid the cost and risk of further litigation.
