Overview:
UMR Inc. has agreed to a $20.25 million class action settlement following allegations it wrongfully denied insurance claims related to emergency room (ER) visits and urinary drug screenings (UDS). The settlement resolves a lawsuit filed by the U.S. Department of Labor.
If you had a health insurance claim denied by UMR under an employer-sponsored plan for either of these services, you may be eligible for a cash payment.
Who Qualifies for a Payment?
You may be eligible if you:
- Were covered under an employer-sponsored health plan administered by UMR Inc.
- Had a claim denied for an emergency room visit because UMR decided it wasn’t a true emergency
OR - Had a urinary drug screening claim denied because UMR said it wasn’t medically necessary
You may qualify for one or both types of claims. Use the settlement website to check your eligibility.
How Much Can You Receive?
Urinary Drug Screening (UDS) Claims:
- Baseline Payment: $68.85 per eligible UDS claim (automatically mailed, no action needed)
- Additional Payment: Up to $103.27 extra (totaling up to $172.12) if you submit proof of higher out-of-pocket costs
Emergency Room (ER) Claims:
- Up to $353.22 per claim if you submit documentation showing you paid out-of-pocket
You can claim multiple eligible UDS or ER claims with proper documentation for each.
How to File a Claim:
- UDS Baseline Payment: No action needed if you received a check (mailed June 17, 2025)
- Additional UDS or ER Payment:
Submit a claim online or by mail using the appropriate PDF form (separate forms for UDS and ER claims)

Required Information and Proof:
To file a claim, you must include:
- Your unique ID number and PIN from the settlement notice
- Proof of out-of-pocket payments (e.g., copies of checks, credit card or bank statements, payment receipts, or provider invoices)
How Will Payments Be Made?
Payments will be sent by check to the address on file or the one you provide on your claim form.
Payout Timeline:
- Baseline UDS checks mailed: June 17, 2025
- Additional UDS and ER claim payments: Expected by January 13, 2026
